We have subsidised RM100 for the spaying of this cat.

Dear Dr. Chan,

It�s me again, Abdurahemsa Mokhtar from Puchong.

I have brought 1 female stray cat from my friend housing block that I suspect are abandoned by their owner to�the clinic�in USJ for the procedure on 14^nd�July 2014. The procedure was done the same day and I collected the cat on the same day.

As such, I would like to apply for neutering subsidy from AnimalCare and hope for your kind consideration on this. �I am pleased to attach here the photos of the cats involved which were taken before and after the procedure together with the completed form and original receipt.

I will pass the original receipt and form to you soon somewhere this week.
