Neutering Aid For 1 Cat In Puchong (Veronica Ong May Yoong�s)

We have sponsored RM90 for the spaying of Lara. We thank Ms Ong for claiming only RM90 for this case. The money will be channeled to other animals.

A very good day Dr Chan!

We have captured a stray female cat which we have been feeding on an open back lane nearby our house.

She has recently gave birth to 3 kittens, which are about 4 months old (1 was killed by dogs and other no where to be seen, no idea what has happened to it).

Sometimes she would even come over to our front porch to wait with the others for food.

Have decided to name her Lara.

Kindly reimburse to us RM90.00 for this claim.

Attached please find the duly signed and completed Neutering Subsidy Form, receipt and photos for your records. These will be sent to you via Pos Ekspres.
