Dear Dr Chan,
I am Hasanah. I know about you from animalcare website.
Last 3weeks, I have found an adult male cat at the grocery shop at Gombak. it looks like it was just ‘thrown away’ by his master because his body was quite clean compared to the other stray cats. I decided to save it after I knew the cat had no master after asking the shop owner. then only last sunday i sent him for ‘spaying’. The reason is I do not want him fighting with other cats at home and urinating everywhere.
Attached herewith the photo of Chucky before & after spay and the receipt from the clinic.
Kindly guide me how to claim and what form to fill up.
