Dear Dr. Chan,
We have taken 2 stray cats, Brave and Koko, to the vet for neutering surgery on 28 April 2015 and would like to apply for neutering subsidy. We attach herewith the scanned copies of the forms, receipts and their before and after surgery photos. The physical copies of the forms and original receipts have been posted and should arrive at your address soon.
